Juniper Community provides community based care programs that assist our aged clients to continue to live independently in their own homes.  Renowned for our commitment to the delivery of person centred care and best practice in relation to the provision of quality care, you will be working in a supportive environment that encourages initiative and leadership.

The Client Care Advisor is responsible for the coordination and delivery of community aged care packages, incorporating the wellness approach and embracing the Consumer Directed Care model. Care and services are to be within budget, responsive, timely and empowering, enabling the client to lead a continued safe and independent value added life within their own home.

 

On offer for the successful candidate is a permanent role, 75 hours per fortnight.

Key Responsibilities:

  •  Development of care plans for clients on aged care packages in collaboration with the client, family and clinical team.
  • Service planning, case management, budget management and administration of Consumer Directed Care packages.
  • Monitor at-risk clients and review service delivery to ensure suitability of services.
  • Attend Community Support Worker, ACAT and Sector Network meetings to build and develop relationships and promote Juniper Community.
  • Identify and resolve operational issues as they arise.
  • Ensure compliance with Occupational, Safety and Health requirements including appropriate Injury Risk Management.

Demonstrated Skills, Knowledge and Experience required:

  • Relevant Diploma or Cert IV qualifications in Community Services, Aged Care or Home and Community Care
  • Coordination and case management of Community based care programs within the consumer directed care model.
  • Experience in managing budgets and interpreting financial information.  
  • Knowledge of Procura is highly desirable.
  • A customer focussed attitude with the ability to build relationships based on trust and respect.
  • Strong client advocacy skills, excellent teamwork and liaison skills.
  • High level computer literacy as well as excellent written and verbal communication skills.
  • A current and valid WA Driver's Licence
