Juniper Community is currently seeking an experienced and professional Physiotherapist to support our elderly Clients living in their own homes. Predominantly you will be engaged to visit our community based clients (65yrs +) to undertake initial assessments, develop appropriate care plans, therapy and exercise programs. You will be required to quickly develop rapport with our clients, liaise with relevant health professionals, family members as well as undertake all documentation within agreed timeframes.

On offer for the successful candidate is a casual position working a variety of hours.

Key Responsibilities of this role:

  • Assess and evaluate Physiotherapy needs and develop care plans that support the maintenance or restoration of our clients’ independence.
  • Ongoing review and evaluation of care plans and therapy programs to ensure the needs of the client are being met.
  • Maintain records, information and complete documentation within the designated time frames.
  • Establish and maintain effective communication and working relationships with clients, colleagues and stakeholders.
  • Ensure compliance with Occupational, Safety and Health requirements including appropriate Injury Risk Management.

The Skills, Knowledge and Experience required:

  • Relevant tertiary qualifications and current AHPRA Registration. 
  • Previous experience working in community or residential aged care will be highly regarded.
  • Ability to relate to elderly clients with empathy whilst working effectively with other health professionals and service providers.
  • Demonstrated success working in an aged care setting including experience in home assessment and equipment prescription.
  • Experience in the development and delivery of therapy based programs.
  • Excellent interpersonal and communication skills.
  • Knowledge of community based programs and a flexible approach to work.
